Public Notice: Westfield Hospital

TO ALL PATIENTS OF WESTFIELD HOSPITAL AND MEMBERS OF THE COMMUNITY

Westfield Hospital located at 4815 Tilghman Street, Allentown, PA 18104 will be closing its doors and no longer providing patient care services or emergency medical services as of 12:01 a.m. December 3, 2013.
Patients wanting a copy of their medical records should contact Westfield Hospita lby telephone at (610) 973-8400 during the hours of 8 a.m. and 4 p.m. Monday through Friday. Information as to how to obtain a copy of the medical records is also available on Westfield Hospital’s website which is www.WestfieldHospital.com.

Westfield Hospital wishes to thank its patients and members of the community for their support over the years and for giving Westfield Hospital the opportunity to provide compassionate care to its patients.
